Within the presentation, the training seminar has been organized for experts who will be involved in implementation of this project in regions. Within two days doctors, medics of an average link, psychologists, social workers, specialists in a healthy lifestyle will master methods of work with the patients involved in this program.
The program of management of diseases in Kazakhstan is realized since 2013. The pilot project has been approved in seven policlinics of the North Kazakhstan and Pavlodar regions. During his realization more than 1,200 patients with arterial hypertension, diabetes and chronic heart failure have been captured.
The program gives to patients the chance together with medics to control the disease and to prevent complications by continuous contact with multidisciplinary team which part the therapist, the profile expert, average medical personnel, the psychologist, the specialist in formation of a healthy lifestyle, the caseworker are.
Considering positive results, the project has been included in the state program of development of health care "Densaulyk", for 2016-2019.
